In case you didn't know already, morning sickness refers to the feelings of recurrent nausea and associated vomiting experienced by a lot of women in early pregnancy. The exact cause is unknown but it is thought to be associated with hormonal changes. Though it can be very unpleasant to the mother to be,the baby is usually fine. Management usually entails a lot of rest and fluid intake,as well as,dietary modifications. A medication like the one mentioned above is however said to be the first FDA approved for home management. World Hepatitis Day 2017 Events are in full swing all around the world and the emphasis is on how we can all work together to "ELIMINATE HEPATITIS". Fortunately, the first GLOBAL HEALTH SECTOR STRATEGY ON VIRAL HEPATITIS by the World Health Organisation (WHO) was formulated to be executed in this regard between 2016 to 2021, towards ending Viral Hepatitis- the most common cause of inflammatory injury to the liver- with a focused longterm agenda of eliminating viral Hepatitis as a major Public Health treat by 2030. It is however important to get a better understanding of: how it all begins from transmission of the disease to infection; the possible effects of especially Hepatitis B & C on the liver and how things can progress if management control measures are not sort after; just to better appreciate why we must all strive to ensure that,as much as possible,we "ELIMINATE HEPATITIS" come 2030.

Though Hepatitis can be caused by chemical assaults on the liver e.g. the longterm effects of alcohol abuse or short to longterm effects of certain drugs, as well as certain autoimmune conditions in which the body for unclear reasons, develops immune cells that attack the liver, majority of cases worldwide occur as a result Hepatitis Virus infections  and emphasis is usually on Hepatitis B & C because of the potential for chronicity (long term infection)/ progression to severe liver damage and cancer. Route of transmission for these 2 viruses would either be via: sharing of needles between  infected and non-infected intravenous drug users; transfusion of infected blood; mother-to-child transmission during delivery in some cases or via sexual intercourse with an actively infected/disease transmitting partner. The body reacts to initial injury to the liver, attempting to repair via inherent physiologic mechanisms that eventually fail,leading to more injury. Symptoms include: greenish-yellow discolouration of the eyes (jaundice) in majority of cases,generalised weakness, general feeling of unwell,nausea and vomiting, mild headaches in some cases +/- low grade increase in body temperature. In some cases, infection can however go unnoticed (asymptomatic) and coincidentally picked up during hospital visits for other complaints. In a number of cases, if the disease is not diagnosed via appropriate testing or ignored after diagnosis for reasons best known to the patient,continued liver injury eventually progresses to severe scarification/liver damage-"Cirrhosis" and then "Liver cancer"  in a percentage of the infected population,as illustrated above. The result of this trend was a whooping 1.34 million deaths worldwide in 2015 alone,from 325 million people identified to be living with chronic Hepatitis. Imagine the unidentified case figure and do a mental estimation of what the world has been dealing with. Now you appreciate the dire need for testing and identification of cases for onward treatment, as prevention/control of spread would even be defective, if gaps continue to exist in testing,diagnosis and treatment. These targets can only be achieved via meaningful collaboration between the public and private stakeholders involved,as well as concerted efforts from members of the general public. The Global Health Sector Strategy on Viral Hepatitis and other policy efforts in the near future will work if as individuals or families,we:
*  present for appropriate testing.

* avoid unsafe sexual practices via -abstinence, being faithful or appropriate use of condoms (ABC)
* make injections safe by avoidance of needle/sharps sharing

* insist on appropriate screening of blood products and confirmation before transfusion

* partake in routine Antenatal care etc.

Essentially what I will be doing today is health promotion. Responsible health promotion must provide three things:

1. Information
2. Reassurance
3. A plan of action.

Let me start with a background on prostate health.

Everyone has a pair of kidneys. The job of the kidney is to remove waste. It is the LAWMA (waste management company) of your body. Everyday your blood passes through the kidney several times to be filtered. As the blood is filtered, urine is formed and stored in a temporary storage tank called the urinary bladder.

If there were to be no urinary bladder, as a man walks on the road, urine will be dropping.

Now think of the plumbing work in your house. Think of the urinary bladder as the overhead storage tank. From the storage tank, a good plumber will run pipes to other parts of the house, including the kitchen. God in His wisdom ran pipes from our urinary bladder to the tip of the penis. The pipe is called the urethra. Just below the bladder and surrounding the urethra is a little organ called the prostate gland.

The prostate gland is the size of a walnut and weighs about 20grams. Its job is to make the seminal fluid which is stored in the seminal vesicle. During sexual intercourse, seminal fluid comes down the urethra and mixes with the sperms produced in the testicles to form the semen. So semen technically is not sperm. It is sperm + seminal fluid. The seminal fluid lubricates the sperm.

After age 40, for reasons that may be hormonal, the prostate gland begins to enlarge in some men. From 20 grams it may grow to almost 100 grams. As it enlarges, it squeezes the urethra and the man begins to notice changes in the way he urinate.

If you have a son under 10, if he has a little mischief like we all did at that age, when he comes out to urinate, he can target the ceiling and the jet will hit target. Call his father to do same, wahala dey. His urine stream is weak, cannot travel a long distance and sometimes may come straight down on his legs. So he may need to stand in awkward position to urinate. Not many men will be worried their urine stream cannot hit the ceiling. Toilets are on the floor and not on the ceiling. But other symptoms begin to show.

* TERMINAL DRIPPLING:

The man begins to notice that after urinating and repacking, urine still drops on his pants. This is the reason why after an older man urinates, he has to ring bell. A younger man simply delivers to the last drop and walks away. Just see an older man coming from the bathroom. Sometimes he may clutch the newspaper closely to hide the urine stains, particularly on plain colored trousers.

* HESISTANCY:

At this point you wait longer for the urine flow to start. There are 2 valves that must open for you to urinate – the internal and external sphincters. Both open but because of obstructions in the urethra, you wait long for the flow to start.

* INCOMPLETE EMPTYING:

You have this feeling immediately after urinating that there is still something left.

As all these things happen, the bladder begins to work harder to compensate for the obstruction in the urethra. The frequency of urination goes up. Urgency sets in. sometimes you have to practically run into the toilet. Nocturia (excessive urination during night time) also becomes common. You wake up more than 2 times at night to urinate. Your wife begins to complain.

Men being men may not talk to anyone even at this point. Then the more serious complications start.

Stored urine gets infected and there may be burning sensation when urinating.

Stored urine forms crystals. Crystals come together to form stone either in the bladder or in the kidney. Stones may block the urethra.

Chronic urinary retention sets in. The bladder stores more and more urine. The size of the bladder is 40 - 60 cl. A bottle of coke is 50cl. As the bladder stores more urine it can enlarge up to 300cl. An overfilled bladder may leak and this leads to wetting / urinary incontinence. Also the volume may put pressure on the kidney and may lead to kidney damage.

What may likely bring the man to hospital is acute urinary retention. He wakes up one day and he is not able to pass urine.

Everything I have described above is associated with prostate enlargement, technically called benign prostate hyperplasia.

There are other diseases of the prostate like:

1. Prostatitis – inflammation of the prostate

2. Prostate cancer – cancer of the prostate.

The bad news is that almost everyman may have prostate enlargement if they live long enough.

The good news is that there are life style changes that can help the man after 40 to maintain optimum prostate health.

* NUTRITION:

Look at what you eat. 33% of all cancers, according to the US National Cancer Institute is related to what we eat.
Red meat everyday triples your chances of prostate disease. Milk everyday doubles your risk. Not taking fruits / vegetables daily quadruples your risk.

Tomatoes are very good for men. If that is the only thing your wife can present in the evening, eat it with joy. It has loads of lycopene. Lycopene is the most potent natural antioxidant.
Foods that are rich in zinc are also good for men. We recommend pumpkin seeds (ugbogulu).
Zinc is about the most essential element for male sexuality and fertility.

Men need more zinc than women. Every time a man ejaculates he loses 15mg of zinc. Zinc is also important for alcohol metabolism. Your liver needs zinc to metabolize alcohol.

*ALCOHOL CONSUMPTION:

As men begin to have urinary symptoms associated with prostate enlargement, it is important they look at alcohol consumption. More fluid in means more fluid out. 

Drink less. Drink slowly.

*EXERCISE:

Exercise helps build the muscle tone. Every man should exercise. Men over 40 should avoid high impact exercise like jogging. It puts pressure on the knees. Cycling is bad news for the prostate. We recommend brisk walking.

*SITTING:
When we sit, two-third of our weight rests on the pelvic bones. Men who sit longer are more prone to prostate symptoms. Do not sit for long hours. Walk around as often as you can. Sit on comfortable chairs. We recommend a divided saddle chair if you must sit long hours.

*DRESSING:

Men should avoid tight underwear. It impacts circulation around the groin and heats it up a bit. While the physiological temperature is 37 degrees, the groin has an optimal temperature of about 33 degrees. Pant is a no - no for men. Wear boxers.  Wear breathable clothing.

*SMOKING:

Avoid smoking. It affects blood vessels and impacts circulation around the groin.

I am not a prophet but I believe strongly that Nigerian and Wolverhampton Goal Keeper,Carl Ikeme, will get into remission from Acute Leukaemia and return to the Goal post. News hit the airwaves over 2 weeks ago that,the 31-year old Goalie was diagnosed with Acute Leukaemia after pre-season blood investigation results revealed what turned out to be abnormally high levels of white blood cells (the component of blood supposedly responsible for fighting against infections).

  Suicide seems to be the easiest way out when individuals reach the rock bottom of their mood scales and coping mechanisms no longer seem to make any meaningful sense. Others are a victim of mental conditions associated with hearing voices,that continually advise them to end it. You probably thought that the only/major predisposing factor was financial lack and all of the frustration that come with it, yes? Could it be that unexpected or poor management of overwhelming success can also lead to a deep loneliness and internal need for greater purpose in the wrong direction, that causes morbid frustration and consequent suicidal ideations? Qualified Psychiatrists would know better but one thing I know for sure is that,you can't boast of being Mr/Mrs Super Immune and certainly need to start seeking professional help,when strange negative thoughts start creeping into your head. Yes,there is a lot to hate about the world and sometimes it truly feels like no one cares. The pressure keeps mounting for whatsoever reason and you can't seem to take it any more longer. Every suicide is a devastating incidence that warrants a debate but the most important aspect for me will be, identifying what exactly went wrong and how to prevent other cases from occurring. #MEMORY LANE: July 20th;Ebola Virus Disease and The Giant of Africa(Nigeria).


July 20th is a day that will never be forgotten in the Healthcare history of Nigeria or the giant of Africa,as many National enthusiasts call her. Today marks 3 years since the deadly hemorrhagic(bloody) Ebola virus found it's way into Nigeria to cause one of the greatest healthcare scare that my generation has ever witnessed. The outcome was 20 cases and 8 very sad deaths,including late Dr Ameyo Adadevo,the Chief of the in-house managing team- now considered to be a National Heroine-for blowing the whistle and stepping her feet down to ensure that quarantine measures were attempted after a high index of suspicion for the index case.

 The virus had been rocking the boats of other neighbouring West African countries dangerously and the death toll was fast climbing. If you were one of those that read my first Blog ever  in early 2014 (drkevwe.com -no longer exists),you would be witness to the fact that myself and some of my colleagues had warned of the possibilities of inter-regional spread and had advised that vital measures be put in place geared towards "emergency preparedness". I can no longer remember if I was the Medical Officer on Call that faithful day, in the popular Hospital I worked in Lagos,just a few minutes from First Consultant Hospital Obalende,where the index case was rushed to straight from the airport but I was particularly touched because fast forward to a few days later, the news of Mr Patrick Sawyer's arrival in Nigeria and consequently Ebola virus disease hit the airwaves and those of us who worked around realised very much that the exposure story and mortality that followed suit could have been any of us:from Senior to Junior clinical staff who would have been involved in that management had the case presented at our professional Door steps.

Hand sanitisers became scarce commodity and the universal precaution hygiene culture of hand washing rose to a sky-top peak, after members of the general public realised that transmission was by coming in contact with body fluids of infected persons and that hand sanitisation at every opportunity could reduce the risk of getting infected. The clinical community went to town with trying to inform members of the general public about signs and symptoms and those of us in the media made sure to disseminate vital information when ever on air. I remember sitting on a discussion panel of an NTA International Programme alongside,Prof. Innocent Ujah, the then Director General of The Nigerian Institute of Medical Research, just to ensure that members of the general public were well informed about the signs and symptoms to enable early and prompt reporting of suspected cases.

The appropriate state/National Health authorities sprang into action in partnership with the World Health Organisation(WHO) and with an utmost sense of responsibility,by the end of August/ beginning of September 2014 and since then, no new cases have been reported.  Major treatment modality for those who survived proved to be mainly supportive,even if a WHO sponsored trial vaccine in Guinea later showed major promise towards prevention,alongside contact racing and appropriate lifestyle modification. Up until today an outright cure has not been established for the disease but vital lessons have been learnt worldwide. Have we stopped washing our hands as a Nation?
